DCS insurance verification provides all data reported by the carrier, including:

• Effective dates for the patient’s active insurance coverage 
• Procedures and treatment categories covered, and those not covered
• The portion of fees the plan pays and the financial obligation of the patient

DCS collects the patient’s plan eligibility and coverage details only. Predetermination and pre-authorizations fall outside this service.

However, if they are part of the carrier’s report, requirements for pre-authorizations are included.

Services do not include:

• Coverage details for specific CPT codes not listed on the carrier’s report
• Past history breakdowns
